University Health & Accident Plan (UHAL)TM/MC for Canadian Students Available only for Ontario residents
While many students now have access to extended health and dental benefits through their students
association, there are many more who do not have access to such benefits.
To that end, the University Health & Accident Plan for Canadian Students is pleased to offer comprehensive
health and dental plans. Until now, individual health and dental plans were too expensive for most students.
Student Options provides affordable and flexible health and dental coverage.
You have your choice of Plan A or Plan B.

Benefits |
| Dental Benefit |
Dental services covering basic expenses including exams, x-rays, fillings and scalings
paid at 50% in year one, 70% in year two, and 80% in years three and up with an annual
anniversary year maximum of $700 per person.

| Drug Benefit |
Generic drugs covered up to a maximum of $250 per anniversary year. If a non-generic
drug is purchased, payment will be based on the lowest generic drug cost equivalent,
if applicable. The benefit is subject to a lifetime maximum of $6,000. No capped
dispensing fee.

| Vision Care |
$100 per 2 consecutive benefit years for eyeglasses, prescription lenses, frames and contact
lenses. Does not include industrial safety glasses.
|
| Counselling Services |
Stress management, emotional problems, learning and behavioural problems, marriage and
family counselling and alcohol or drug abuse are areas covered when rendered by registered
psychologists. Does not include smoking cessation therapy, academic or scholastic testing,
or career counselling. $75 maximum for initial visit, $60 maximum for subsequent visit. Maximum
10 visits in any anniversary year.

| Registered Specialists and Therapists |
Payment is based on reasonable and customary payment for services rendered.
Services covered include:
- Massage Therapist (requires doctor's referral): $15 maximum per visit,
maximum 15 visits per anniversary year
- Speech Pathologist (requires doctor's referral): $60 maximum for initial
visit, $40 maximum for subsequent visit, maximum 10 visits per anniversary year.
- Physiotherapist: maximum $250 per anniversary year
- Osteopath: $15 maximum per visit, maximum 20 visits per anniversary year
- Podiatrist: $15 maximum per visit, maximum 20 visits per anniversary year
- Naturopath: $15 maximum per visit, maximum 20 visits per anniversary year
- Chiropodist: $15 maximum per visit, maximum 20 visits per anniversary year
- Chiropractor: $15 maximum per visit, maximum 20 visits per anniversary year

| Home Support Services |
This covers the services of a registered health professional including a registered nurse,
registered nursing assistant, or health care aid to provide medical care in your home.
Prior approval is required. Maximums per year:
- First: $500
- Second: $800
- Third: $1,000
- Fourth: $1,200
- Fifth and Over: $2,500

| Medical Equipment and Supplies |
Surgical bandages and dressings and the purchase or rental of medically necessary
equipment such as crutches, non-electric wheelchairs and hospital beds, oxygen and
other equipment recommended by your physician and approved by Manulife Financial.
Maximum coverage per year:
- First: $500
- Second: $800
- Third: $1,000
- Fourth: $1,200
- Fifth and Over: $2,500

| Accidental Dental |
Dental treatment required as a result of an accidental blow to the head or
mouth. Treatment must begin within 6 months of injury and Manulife Financial must
be notified within 90 days. $2,000 maximum per anniversary year.

| Accidental Death and Dismemberment |
Payment for a loss directly resulting from accidental bodily injury or accidental
loss of life occuring within a year of the date of the accident. $10,000 maximum
for subscriber or spouse; $4,000 maximum for child.

| Ambulance Services |
Covers trips to hospitals in a licensed ground ambulance. Covers charges up to
the amount between reasonable and customary charges and your Provincial Health
Plan allowance, if applicable.

| Travel Benefit |
Protection for emergency medical expenses while travelling outside the province.
- Emergency hospital and medical expenses
- 24-Hour worldwide medical assistance
- Unlimited number of trips of up to 15 days each
- No age maximum
- Does not cover pre-exiting conditions within the past 9 months.
$100 deductible on this portion of the plan.
